Patient Care

Our commitment to comprehensive care provides patients with full access to a broad range of services. Whether an endocrine disorder is a common one or more rare and complex, our experienced specialists ensure each patient receives compassionate care and efficient, effective treatment. Our services include:

  • Expert medical care for the full spectrum of endocrine disorders and conditions, including access to advanced imaging and endocrine testing and referral to experienced endocrine surgeons when needed;
  • Specialized programs in adrenal disease, thyroid cancer and osteoporosis;
  • Diabetes treatment by a multidisciplinary team of endocrinologists, advanced practice clinicians (nurse practitioners, physician assistants), dietitians and behavioral health specialists who work together to provide comprehensive and patient-centered care;
  • Extensive experience with advanced diabetes technology, including insulin pumps, continuous glucose monitors and automated insulin delivery systems;
  • High quality diabetes education provided in individual sessions and through our ADA-recognized diabetes education program (PROMISED - see below);
  • Behavioral specialists who offer individualized treatment (cognitive behavioral therapy) for health-related distress, depression and anxiety and conduct support groups where patients can share their experiences and coping strategies;
  • The SEAD program which provides specialized care for young adults with type 1 diabetes, including services and activities to address developmental and psychosocial needs of this vulnerable age group.

PROMISED

Montefiore's unique diabetes self-management group education program, the Proactive Managed Information System for Education in Diabetes (PROMISED©) is nationally recognized by the American Diabetes Association and frequently cited for excellence. Providing high-quality education in a relaxed and supportive atmosphere is a primary goal of the program. The PROMISED program is for adults with type 2 diabetes and taught in a group setting by highly qualified nurses and nutritionists who specialize in diabetes. The program consists of five weekly 2-hour sessions, currently conducted remotely via Zoom. Classes are conveniently scheduled during morning or evening hours and are offered in both English and Spanish. Topics include:

  • Healthier eating
  • Becoming more active
  • Diabetes medications – how they work and how to take them
  • Coping with stress
  • Making positive changes
  • Getting the best care from providers
