Teaching evaluations should be included in the Teaching Portfolio, especially those that are on file with the Office of Medical Education (OME). If the OME does not have evaluations on file, then evaluations from another source should be provided. For applications in the Investigator Track, Physician Investigator Track, Research Professor Track and Associate Track, inclusion of a Teaching Portfolio is highly recommended.

Applications and supporting documents should be sent to the Department of Medicine’s Office of Faculty Affairs for review and Chair approval. Please note, application material should be typed, one-sided, binder clipped (not stapled) and filled out completely.
